All of our patientís Health & treatment information is treated in a secure, respectful and fully confidential manner. It is important that we have accurate and up to date records about your health and treatment so that we can give the best possible advice and care . This information is only available to those involved in your care. You have a right to know what information we hold about you, and if you would like to see your records please contact the Practice Manager. 

CLINIC INFORMATION
GENERAL NURSING
For general nursing, including travel vaccinations, ear syringing, blood tests, registration medicals, advice on diet, smoking, alcohol intake etc.

MATERNITY/MOTHER & CHILD
We offer full antenatal and postnatal care with Doctor & Midwives by appointment.

IMMUNISATION & CHILD HEALTH
We provide advice, childhood immunisations and development checks with the Doctor & Health Visitor by appointment.

WELL BABY
Our Health Visitor has weekly clinics for weighing, discussions on feeding and general care at the Old School Surgery, and monthly at Chilham Surgery.

HEALTH PROMOTION
A range of health checks and health promotion sessions are available to both men and women run by the Practice Nurse and include:
Advice on: - smoking, weight, exercise, heart disease & strokes, diet and alcohol.
Screening: Blood pressure, blood tests & cholesterol.

FAMILY PLANNING/WELL WOMAN
This Clinic is run by our Practice Nurse Brenda Smith who is specially qualified in Family Planning, Well woman screening and hormone replacement therapy.
∑ We are able to supply and give advice on all forms of contraception, including cap and coil fitting.
∑ Advice on pre-menopausal problems and HRT is available.
. Well woman screening includes cervical smears, breast examination, blood pressure checks, urine testing and other appropriate health advice.

SMEARS: Cervical smears are a vital aspect of health care and we recommend that women have regular smears every 3 years, between the ages of 20-65 years. Appointments for these may be made with the Practice Nurse or Doctor. If you wish to have a chaperone, please indicate your preference at time of making appointment.

DIABETIC/ASTHMA
These specialist clinics are run regularly by our Practice Nurses, to monitor and follow up the long term progress of people with asthma and diabetes.

TRAVEL & IMMUNISATION
We are a Specialist Travel Immunisation Centre, offering all travel vaccines, including Yellow fever, Rabies & Meningitis.

MINOR SURGERY/CRYOTHERAPY
We are registered and qualified to perform Minor surgery.
Our premises are fully equipped for this and procedures are available by appointment after prior consultation with the Doctor.

HOW TO SEE YOUR DOCTORS
All consultations with the Doctors are by appointment only. Please contact the appropriate surgery. Urgent cases will always be seen on the same day.

If you wish to speak to the Doctor or Nurse, you may be asked to phone back after the Surgery has finished, or if appropriate they will contact you when free.
If you are unable to keep an appointment, please let us know.

HOME VISITS
If you need to see a Doctor, but are unable to attend the Surgery for medical reasons, please telephone either surgery between 8.30-10.30am (if possible) to request a house call. The Receptionist will need your name, address and telephone number, together with brief details of the problem. This enables the Doctor to prioritise his calls.

OUT OF HOURS CALLS
If you need advice or to see your Doctor outside of normal surgery hours, please telephone either surgery and further instructions will be given to you via a recorded message.

DISPENSARY - PRESCRIPTIONS
We are a Dispensing practice. Both Old School Surgery and Chilham Surgery have their own dispensaries which are open during surgery hours.
Patients may obtain their medicines from us provided that they do not live within one mile of a Chemist

REPEAT PRESCRIPTIONS
Whenever possible, please order repeat prescriptions 48 hours in advance. These may be collected during normal Surgery hours after 48 hours.

HOW TO REGISTER
Please bring your medical card to the Surgery if possible. All newly registered patients will be asked to complete a simple questionnaire and to see the Practice Nurse for a New Patient Registration Health Check.

CHANGE IN CIRCUMSTANCE
We would appreciate notification of any change in address including post code, telephone number, marital status or name, as soon as possible, so that our records can be kept up to date.

NON NHS SERVICES
Certain medical examinations do not come within the jurisdiction of the NHS, and there is a charge for these services in accordance with the BMA recommended scales.

These include HGV, Pre-employment, Elderly Drivers etc. There is also a fee payable for signing Private Medical Insurance claims, Cancellation of Travel, Shot Gun Certificate and Passports.

DISABLED ACCESS
There is access and toilet facilities for the disabled at the Old School Surgery, Chartham. Disabled access at Chilham is via a purpose built ramp.
